Indemnification by Parent or Guardian of Applicant: I understand that injury is a possibility in the participation and/or training for any athletic activity. I approve of my child’s attendance and participation in the Motion Concepts Sports Camp LLC. I certify that my child is physically fit to participate. In further consideration of the Motion Concepts Sports Camp LLC and the Harford County Parks and Recreation, by accepting this application, I/we hereby agree to save and indemnify and keep harmless the Motion Concepts Sports Camp, its agents or sponsors against any and all liability claims, judgements or demands for damages arising as a result of any course given by coaches and staff. Motion Concepts Sports reserves the right to use any photographs taken for the purpose of current or future promotional endeavors. This could include but is not limited to brochures, local papers, and the motionconcepts.net web site. Online registration indicates acceptance of indemnification.
